Virtual Medical Receptionist: 2026 Prices Compared

A virtual medical receptionist answers a practice's patient calls remotely: scheduling and rescheduling appointments, confirming visits, routing urgent calls, requesting records and updating the patient system. As of 2 November 2026, published prices fall into four models. AI medical receptionists (e.g., MedPhone, MedReception AI): $150 to $495 a month for 500 minutes. Medical answering services (e.g., notifyMD): $155 for 100 minutes to $685 for 500. General live receptionist services (e.g., PATLive, Abby Connect, Ruby): $189 to $395 for 100 minutes. Dedicated remote medical receptionists billed by the hour (e.g., Hello Rache at $9.50 an hour, Portiva from about $10). Remote staffing agency, Latin America in US time zones (e.g., Virtustant): a dedicated, bilingual medical receptionist from $7 an hour all-in, about $560 a month at 20 hours a week and $1,120 full-time, with zero placement fees.
This guide compares 11 options by their published prices and what each one says about HIPAA, works out what each costs at 200 to 2,000 minutes a month, and gives you the Three-Line Front Desk Test for splitting a practice's calls between AI, an answering service and a person. It also shows what 19 medical searches in Virtustant's recruitment records asked for. Virtustant is a remote staffing agency and one of the options compared, so we stick to published facts and show every calculation. This is not legal advice on HIPAA.
The job is the front desk without the desk. What changes between providers is how much of it they take on, and whether the person is shared across many practices or works only for yours.
| Front-desk task | AI receptionist | Shared answering service | Dedicated remote receptionist |
|---|---|---|---|
| Answering and routing calls | Yes, 24/7 | Yes, usually 24/7 | During the hours you schedule |
| Booking and rescheduling | Inside supported EHR integrations | Often message-based or in a supported calendar | Directly in your EHR or practice system |
| Urgent calls and on-call escalation | Rules-based routing | A core service of medical answering services | Follows your protocol during the shift |
| Records requests, faxes, referrals | Some plans add these as modules | Rarely | Part of the job |
| Insurance and benefits questions | Scripted | Message taken | Handled with access you grant |
| Spanish-speaking patients | Depends on the product | Often available | Native Spanish when hired from Latin America |
The rows below the first two are where most practices find the difference. For the full list of front- and back-office tasks, see what a healthcare virtual assistant does.
Prices were read on each provider's own page between 29 October and 2 November 2026. Where a page does not publish a figure, the table says so rather than estimating it.

| Virtustant (remote staffing agency) | Dedicated bilingual medical receptionist | From $7 an hour all-in: about $560 at 20 hours a week, $1,120 full-time | Month-to-month; zero placement fees; one weekly invoice per placement |
The four models price four different things. AI plans price minutes cheaply. Answering services price minutes of a shared human. Hourly services and staffing agencies price a person's time, whatever the call volume. That is why the cheapest option at 200 minutes a month is rarely the cheapest at 2,000.
The table uses the cheapest published plan for each volume plus published overage. At about 2.5 minutes a call, which is our assumption, 200 minutes is about 80 calls a month and 2,000 minutes about 800. Abby Connect's overage is assumed at its plan's own minute rate, as its pricing page describes.
| Minutes a month (about calls) | MedPhone AI | MedReception AI | notifyMD | PATLive | Abby Connect | Ruby | Dedicated receptionist, $7/hr |
|---|---|---|---|---|---|---|---|
| 200 (80) | $150 | $495 | $291 | $349 | $599 | $720 | $560 part-time / $1,120 full-time |
| 500 (200) | $150 | $495 | $685 | $759 | $1,380 | $1,725 | $560 / $1,120 |
| 1,000 (400) | $300 | $995 | $1,350 | $1,499 | $2,760 | Not published | $560 / $1,120 |
| 2,000 (800) | $600 | $1,495 | $2,680 (est., over 500) | $2,899 | $5,520 (est.) | Not published | $560 / $1,120 |
Two things stand out. AI is by far the cheapest way to buy minutes: MedPhone's 2,000 minutes cost less than one week of a full-time person. At 500 minutes, human per-minute services cost 4.6 to 11.5 times what MedPhone AI does, and they pass the cost of a full-time seat between about 370 and 830 minutes a month. The 2,000-minute figures for notifyMD and Abby Connect extend their top published plan at its overage rate; both say to ask for a quote at that volume.
Under the HIPAA Privacy Rule, a practice that lets a vendor handle protected health information generally needs a business associate agreement with that vendor first. A badge on a website is not that agreement. Here is what each provider's own pages said when we read them:
| Provider | What its page says | Business associate agreement on the page |
|---|---|---|
| MedReception AI | “HIPAA compliant, verified by Compliancy Group” | “BAA included” |
| MedPhone AI | “HIPAA compliant on every plan”; PHI encrypted at rest and in transit | Not mentioned |
| notifyMD | “The First HITRUST Certified Answering Service” | Not mentioned on the pricing page |
| WellReceived | Each team member “trained in HIPAA compliance” | Not mentioned |
| Hello Rache | “HIPAA-Trained & Certified” | Not mentioned |
| Portiva | Assistants “HIPAA-trained” before onboarding | Not mentioned |
| MEDVA | “Every MEDVA Virtual Medical Receptionist is HIPAA-trained” | Not mentioned |
| Virtustant | Your practice sets the HIPAA rules and controls the access; the receptionist works only inside the systems you grant | Scoped on the discovery call against your EHR and privacy requirements |
Training and agreements are different things. Ask every option in writing whether it will sign your business associate agreement, where call recordings and transcripts are stored, and who can see them. For a dedicated receptionist, the access controls are yours: role-based logins in the EHR, no patient data in personal email, and the minimum access the job needs.
Most practices do not need one answer to the phone. They need three, because their calls are three different jobs. We call this the Three-Line Front Desk Test: sort a week of calls into three lines, then buy the cheapest thing that does each line well.
| Line | What the calls are | Cheapest thing that does it well | Published price example |
|---|---|---|---|
| 1. Routine | Hours, directions, confirmations, simple reschedules | An AI receptionist connected to your scheduling | MedPhone AI: $150 for 500 minutes |
| 2. After hours | Urgent symptoms, on-call paging, messages for the morning | A medical answering service with on-call escalation | notifyMD: $155 for 100 minutes |
| 3. Work after the call | New-patient intake, records requests, faxes, referrals, insurance questions, booking in the EHR, Spanish-speaking patients | A dedicated receptionist working your daytime hours | Virtustant: from $7 an hour all-in; Hello Rache: $9.50 an hour |
The rule of thumb: if line 3 fills more than 20 hours a week, a dedicated person is the cheaper buy for that line, and lines 1 and 2 shrink to whatever volume is left. If it fills less than 5 hours a week, keep it on a shared service.
The crossover volume is the number of minutes a month at which the cheapest published plan costs more than a dedicated seat.
| Option | Crossover vs 20 hours a week ($560) | Crossover vs full-time ($1,120) |
|---|---|---|
| Abby Connect | About 170 minutes | About 373 minutes |
| Ruby | Past 100 minutes | Past 200 minutes |
| PATLive | About 342 minutes | About 701 minutes |
| notifyMD | About 401 minutes | About 827 minutes |
| MedReception AI | About 552 minutes | About 1,126 minutes |
| MedPhone AI | About 1,866 minutes | About 3,733 minutes |
A worked example from our own records: a recent medical receptionist search describes “approximately 20–40 patients per day.” At 21.7 working days a month and 2.5 minutes a call, that is about 1,083 to 2,167 minutes. On published prices that volume costs about $1,460 to $2,902 a month at notifyMD and $1,639 to $3,173 at PATLive, against $1,120 for a full-time receptionist. MedPhone AI would cost about $325 to $650, which is why the honest answer for routine calls at that volume is AI, and why the person should be doing the intake, records and follow-up work that AI and answering services leave behind.
The general answering-service math, without the medical layer, is in our answering service cost guide and virtual receptionist pricing guide.
We pulled every position in Virtustant's recruitment system whose title contains “medical” or “patient”: 19 positions opened between October 2025 and October 2026.

Three posts read in full show the front-office work. A remote medical receptionist for a group of three medical offices answers high-volume inbound calls, schedules family medicine and pediatric visits, processes digital faxes and requests medical records. A medical receptionist supporting law offices that work with clinics makes outbound calls to schedule 20 to 40 patients a day and keeps patient records current. A patient care coordinator for a senior care navigation service handles refills, calls doctor offices and pharmacies, and works inside a case management system. All three ask for English and Spanish. The three posts drew 42, 152 and 157 applicants. Client names, rates, budgets and salaries from these records are not published.
The pattern matches the Three-Line Test: buyers hire a person for line 3. None of the front-office posts asked for overnight coverage.
The US Bureau of Labor Statistics puts the median pay for receptionists at $18.27 an hour in May 2025, and for medical secretaries and administrative assistants at $45,930 a year, about $22.08 an hour, before payroll taxes, benefits, equipment and recruiting. Virtustant's $7 starting rate is about 38% of the receptionist median and about 32% of the medical secretary median. Spanish matters at the front desk: about 44.9 million US residents age 5 and older spoke Spanish at home in 2024, according to the Census Bureau's American Community Survey as summarized by USAFacts.
How the rate works: if you pay your agent $10/hr, we give them $9 and keep the dollar in the middle. The rate is all-in, scoped on the discovery call; see Virtustant pricing.
Virtustant is a remote staffing agency that places vetted medical receptionists, schedulers, patient coordinators and other remote professionals from Latin America with US practices, from $7/hr all-in. Each placement is one named person, most often native in Spanish with C1 to C2 English, working the US hours you set inside the systems you grant. Virtustant accepts the top 1% of applicants: 100% apply, 22% clear the recruiter screen, 9% pass cognitive (IQ), skills and English testing, 3% reach a live interview, 1% are hired. You get a shortlist of 3 to 5 vetted candidates within 48 hours of your first call, and your hire can start in 72 hours. Terms are month-to-month, with zero placement fees and a lifetime replacement guarantee with no time cap.
What it does not do: a dedicated receptionist covers the hours you schedule, not 24/7, and does not replace a medical answering service for overnight on-call paging. See the healthcare virtual assistant role, Virtustant for healthcare practices, or the virtual receptionist role. Dental offices have their own guide: dental virtual assistant.
A virtual medical receptionist answers a practice's patient calls remotely, schedules and reschedules appointments, confirms visits, routes urgent calls and updates the patient system. It can be an AI product, a shared answering service or a dedicated remote person who works only for one practice.
As of 2 November 2026, AI medical receptionists publish $150 to $495 a month for 500 minutes (MedPhone AI, MedReception AI), medical answering services $155 for 100 minutes to $685 for 500 (notifyMD), and live receptionist services $189 to $395 for 100 minutes (PATLive, Abby Connect, Ruby). Dedicated remote receptionists are billed by the hour: Hello Rache publishes $9.50, and a remote staffing agency such as Virtustant starts at $7 an hour all-in, about $1,120 a month full-time.
It depends on the agreement and the setup, not the label. Under the HIPAA Privacy Rule, a practice that lets a vendor handle protected health information generally needs a business associate agreement first. Of the providers compared, MedReception AI publishes that a BAA is included; the others describe HIPAA training or certification but do not publish a BAA statement on the pages we read. Ask every option in writing.
Per minute, yes, and by a lot. MedPhone AI publishes 2,000 minutes for $600 a month, while a full-time dedicated receptionist starts at $1,120. A person is the better buy for work after the call, such as intake, records requests, referrals, insurance questions and Spanish-speaking patients, which AI and answering services handle only partly.
Past the crossover volume. On published prices a full-time receptionist at $1,120 a month costs less than Abby Connect above about 373 minutes a month, PATLive above about 701, notifyMD above about 827 and MedReception AI above about 1,126.
It sorts a practice's calls into three lines: routine calls for an AI receptionist, after-hours and urgent calls for a medical answering service, and work after the call, such as intake, records and EHR booking, for a dedicated receptionist. If line 3 fills more than 20 hours a week, a person is the cheaper buy for it.
Many can. Most Virtustant receptionists are hired from Latin America and are native Spanish speakers with C1 to C2 English, and all three front-office posts we read in our records asked for English and Spanish.
Through Virtustant, you get a shortlist of 3 to 5 vetted candidates within 48 hours of your first call, and your hire can start in 72 hours, with zero placement fees and month-to-month terms.
Third-party prices. Read on each provider's own page: MedPhone AI pricing, MedReception AI pricing, notifyMD pricing, Hello Rache pricing, Portiva, MEDVA virtual medical receptionist and WellReceived virtual medical receptionist on 2 November 2026; PATLive pricing, Abby Connect pricing and Ruby pricing on 29 October 2026. HIPAA wording is quoted from the same pages. Figures are reported as published; prices and terms change, so confirm them in writing.
Calculations. Cost at a volume: the cheapest published plan for that number of minutes plus overage at the published rate. Abby Connect overage: plan price ÷ included minutes. Ruby: plan tiers only, no overage published. Minutes per call: 2.5 (our assumption). Working days: 52 × 5 ÷ 12 = 21.7 a month. Crossover: the highest whole number of minutes at which the cheapest plan still costs no more than the seat. Virtustant monthly cost: $7 × 80 hours = $560 at 20 hours a week; $7 × 160 hours = $1,120 full-time.
Virtustant data. Every position in Virtustant's recruitment system whose title contains “medical” or “patient”, opened between 9 October 2025 and 30 October 2026, read on 2 November 2026.
